PT OT Other 2022 Quantitative measurement of resistance force and subsequent attenuation during passive isokinetic extension of the wrist in patients with mild to moderate spasticity after stroke. Journal of neuroengineering and rehabilitation A custom wrist device measured spasticity in 17 stroke patients. It found no link between peak resistance and clinical scores, but the rate of force drop-off correlated with severity, especially when finger spasticity was included in the assessment. PT OT RCT 2023 Estimating the cost consequence of the early use of botulinum toxin in post-stroke spasticity: Secondary analysis of a randomised controlled trial. Clinical rehabilitation Early botulinum toxin after stroke did not raise total NHS costs over six months. Contracture-related costs were lower, but overall costs did not differ. PT OT Cohort 2023 Responsiveness and trajectory of changes in the rating of everyday arm-use in the community and home (REACH) scale over the first-year post-stroke. Clinical rehabilitation About 87% of participants used their affected arm more by 12 months, with the biggest gains by three months. People with severe baseline arm weakness started lower and improved differently. Arm-use scores changed with arm capacity and self-reported change. PT OT Cohort 2022 Results from the International Spinal Cord Injury Community Survey: The Lived Experience of People with Spinal Cord Injury in South-Eastern Brazil. Journal of rehabilitation medicine People with spinal cord injury in south-eastern Brazil reported spasticity as their most problematic health condition (33%) and neuropathic pain (13%). Public spaces and long-distance transport were major environmental barriers. PT OT SLP Survey 2022 Pandemic intake questionnaire to improve quality, effectiveness, and efficiency of outpatient neurologic and developmental care at the Kennedy Krieger institute during the COVID-19 pandemic. Frontiers in rehabilitation sciences Guardians of children with neurodevelopmental disorders reported worsened mood, behavior, sleep, and therapy-access problems during the pandemic. A pre-visit questionnaire surfaced these concerns, and Five out of six program directors found it helpful. OT Pilot 2022 Improving sleep apnea treatment adherence after traumatic brain injury: A nonrandomized feasibility study. Rehabilitation psychology Thirteen of seventeen veterans with brain injury and sleep apnea completed a four-module telehealth program to improve CPAP use. Retention and therapeutic alliance were high. Sleepiness and fatigue scores did not change significantly. The study tested feasibility, not whether the treatment works. PT OT Pilot 2022 Effects of nordic walking exercise on gait, motor/non-motor symptoms, and serum brain-derived neurotrophic factor in individuals with Parkinson's disease. Frontiers in rehabilitation sciences Twelve people with Parkinson's disease improved their walking endurance and motor symptoms after six weeks of Nordic walking. These gains were still present three months later when they continued exercising on their own, and no falls occurred during the study. PT OT Narrative Review 2022 Utility of transcranial magnetic stimulation in the assessment of spinal cord injury: Current status and future directions. Frontiers in rehabilitation sciences TMS can objectively measure corticospinal connectivity and silent periods in spinal cord injury, detecting residual function missed by clinical exams. However, inconsistent findings, high measurement error, and unknown clinical meaning limit its current use for guiding individual therapy. PT OT Cohort 2023 Age at Onset of Spinal Cord Injury is Associated with Increased Inpatient Care Needs, Reduced Independence at Discharge and a Higher Risk of Institutionalization after Primary Inpatient Rehabilitation. Journal of rehabilitation medicine Older age at spinal cord injury onset predicts more daily nursing hours and lower independence at discharge, but does not change length of stay or daily therapy time. For each additional year of age, the odds of needing institutional care after discharge rise by 1.03-fold. PT OT RCT 2023 Effects of transcutaneous electrical acupoint stimulation on upper-limb impairment after stroke: A randomized, controlled, single-blind trial. Clinical rehabilitation Adding transcutaneous electrical acupoint stimulation to standard stroke rehabilitation did not improve upper limb motor recovery more than sham stimulation. The primary outcome showed no significant difference between groups. PT OT SLP Other 2022 Early, Intense Rehabilitation Fails to Improve Outcome After Intra-Striatal Hemorrhage in Rats. Neurorehabilitation and neural repair Early, intense rehabilitation failed to improve skilled reaching or reduce brain injury in rats with striatal hemorrhage. Neither timing nor duration of treatment provided benefit, challenging the assumption that high-intensity early rehab drives recovery in this specific injury model. PT OT SLP Cohort 2022 Post-concussive symptoms mediate the relationship between sleep problems and participation restrictions among veterans with mild traumatic brain injury. Frontiers in rehabilitation sciences Sleep problems link to participation restrictions in veterans with mild TBI, but the path depends on the symptom type. Cognitive symptoms mediate both social and productivity restrictions. Mood symptoms affect only social participation, while vestibular symptoms affect only productivity. Screen for sleep issues to target these specific barriers. PT OT Other 2022 Limb accelerations during sleep are related to measures of strength, sensation, and spasticity among individuals with spinal cord injury. Journal of neuroengineering and rehabilitation Sleep movement data from ankle accelerometers predicted strength and sensation scores with high accuracy. This suggests passive overnight monitoring could offer a new, unobtrusive way to assess impairment in spinal cord injury without requiring active patient participation. OT SLP Systematic Review 2023 Education on the consequences of traumatic brain injury for children and adolescents with TBI and families/caregivers: a systematic scoping review. Brain injury Pediatric TBI education mostly reached caregivers, not children alone. Written discharge instructions were common, and adding verbal or multimedia support appeared better for recall. PT OT Pilot 2022 The Effects of an Individualized Smartphone-Based Exercise Program on Self-defined Motor Tasks in Parkinson Disease: Pilot Interventional Study. JMIR rehabilitation and assistive technologies Fifteen patients with Parkinson disease completed a 4-week home exercise program via a smartphone app. Usability was good and adherence was high. Patients reported improved personal daily tasks, but standard clinical measures showed no significant change. PT OT Qualitative 2023 The impact of the COVID-19 pandemic on individuals living with spinal cord injury: A qualitative study. Rehabilitation psychology Veterans with spinal cord injury reported losing routine care, physical activity, and social contact during the pandemic. They cited closed facilities, lack of home help, and fear of exposure as barriers, while finding telehealth convenient for some services. PT OT Qualitative 2022 Therapists' perspectives on using brain-computer interface-triggered functional electrical stimulation therapy for individuals living with upper extremity paralysis: a qualitative case series study. Journal of neuroengineering and rehabilitation Therapists who delivered BCI-triggered electrical stimulation therapy in research settings said it would not fit clinical use as delivered. They wanted faster setup, fewer people in the room, and a peer community; they felt physically capable and motivated. PT OT SLP Other 2022 Gaze-contingent display technology can help to reduce the ipsilesional attention bias in hemispatial neglect following stroke. Journal of neuroengineering and rehabilitation Gaze-contingent masks shifted neglect patients' attention leftward. However, this did not improve target detection in the neglected side and worsened reaction times on the right. The technology alters eye movements but does not yet fix functional neglect. OT Case-Control 2022 Functional recovery outcomes following acute stroke is associated with abundance of gut microbiota related to inflammation, butyrate and secondary bile acid. Frontiers in rehabilitation sciences In 12 stroke patients, gut bacteria linked to inflammation and butyrate differed from controls and showed links to self-care, thinking, self-efficacy, support and pain. This is association only; it does not show changing gut bacteria improves recovery. PT OT SLP RCT 2023 Actor-partner effects of wellbeing, hope and self-esteem on depression in stroke survivor-caregiver dyads: A randomized controlled trial. Clinical rehabilitation Narrative therapy groups lowered depression in stroke survivors more than standard psychoeducation did. Survivors’ own wellbeing and self-esteem drove their mood, but their caregivers’ depression also tracked with how the survivor felt, so treating the pair as a unit matters. PT OT RCT 2022 Developing guiding principles for technology-based rehabilitation program by engaging people with motor incomplete tetraplegia. Journal of neuroengineering and rehabilitation People with chronic motor incomplete tetraplegia said they would only stick with home technology if it matched their specific goals, allowed independent setup, and offered a just-right challenge. They rejected devices that were too easy, too hard, or required a caregiver to assemble. PT OT Narrative Review 2022 Brain-Computer Interface-Controlled Exoskeletons in Clinical Neurorehabilitation: Ready or Not? Neurorehabilitation and neural repair Non-invasive brain-computer interface exoskeletons are technically ready but not yet routine care. Current systems have limited accuracy and cannot restore daily living tasks. Clinicians should view them as promising tools for severe paralysis, not proven treatments, while awaiting better devices and reimbursement. PT OT RCT 2022 Augmented reality for stroke rehabilitation during COVID-19. Journal of neuroengineering and rehabilitation Chronic stroke survivors who replaced part of their usual therapy with augmented reality training showed significant improvements in upper and lower limb motor function. However, walking ability did not improve significantly, and balance gains were inconsistent between home and centre-based groups. PT OT Cohort 2023 Energy expenditure and perceived effort during uphill and downhill walking in people with multiple sclerosis. European journal of physical and rehabilitation medicine People with mild multiple sclerosis felt uphill and downhill walking was much harder than healthy controls did, even though their actual energy use was lower. They may be working differently to manage slopes. Ask about effort, not just speed. PT OT RCT 2022 Performing a shortened version of the Action Research Arm Test in immersive virtual reality to assess post-stroke upper limb activity. Journal of neuroengineering and rehabilitation A shortened immersive VR ARAT tracked traditional paretic-hand scores in people after stroke, but omitted six items and needed headset setup. PT OT SLP Meta-Analysis 2023 Healthcare professionals' perceived barriers and facilitators of implementing clinical practice guidelines for stroke rehabilitation: A systematic review. Clinical rehabilitation Time and resources were the most consistent issue in stroke guideline implementation. Competing tasks, poor protocols and low confidence blocked use. Protected time, local routines, training, audit and champions helped. PT OT Other 2022 Berg Balance Scale is a Valid Measure for Plan Interventions and for Assessing Changes in Postural Balance in Patients with Stroke. Journal of rehabilitation medicine A new scoring template for the Berg Balance Scale helps therapists spot the 'just right' challenge level for stroke patients. Items in the transition zone between easy and hard tasks guide short-term goals, while harder items suggest long-term targets. PT OT Systematic Review 2022 Technology-assisted assessment of spasticity: a systematic review. Journal of neuroengineering and rehabilitation No technology-assisted spasticity measure is fully validated for clinical use yet. However, manual stretching with EMG shows promise as a quick, low-cost alternative to the Modified Ashworth Scale. PT OT Pilot 2022 Feedback control of upright seating with functional neuromuscular stimulation during a reaching task after spinal cord injury: a feasibility study. Journal of neuroengineering and rehabilitation Feedback-controlled electrical stimulation helped four of five people with spinal cord injury return to an upright seated position faster after reaching for a weighted jar. Effects on trunk sway and reach distance varied widely between individuals, and one person with a lower-level injury saw no benefit. PT OT Other 2022 Naturalistic visualization of reaching movements using head-mounted displays improves movement quality compared to conventional computer screens and proves high usability. Journal of neuroengineering and rehabilitation Head-mounted displays improved 3D reaching quality and reduced movement onset time compared to 2D screens in healthy elderly and pilot brain-injured patients. Usability was high across all technologies, with no significant differences in motivation or self-reported cognitive load. OT Other 2023 Determining the Marginal Cost Differences of a Telehealth Versus an In-person Occupational Therapy Evaluation Session for Stroke Survivors Using Time-driven Activity-based Costing. Archives of physical medicine and rehabilitation Telerehabilitation stroke evaluation sessions were estimated to cost $231.64 versus $168.29 for outpatient sessions, mainly because of scheduling calls and documentation. PT OT Cohort 2023 Sensor based assessment of turning during instrumented Timed Up and Go Test for quantifying mobility in chronic stroke patients. European journal of physical and rehabilitation medicine A wearable sensor on the lower back during a Timed Up and Go test distinguished stroke patients with limited mobility from those with good mobility. Average turning speed below 39 to 42 degrees per second reliably identified the impaired group, offering a quick objective measure for clinical assessment. PT OT Other 2022 Design and validation of a multi-task, multi-context protocol for real-world gait simulation. Journal of neuroengineering and rehabilitation A short lab protocol with varied tasks captures a wider range of walking speeds than straight walking alone. Adding complex tasks like turning and stepping over obstacles slowed people down more than simply asking them to walk slowly. The protocol was safe for people with mobility impairments. OT SLP Qualitative 2022 Facebook Experiences of Users With Traumatic Brain Injury: A Think-Aloud Study. JMIR rehabilitation and assistive technologies Eight adults with chronic TBI used Facebook for the same social goals as uninjured peers, but cognitive barriers made the experience frustrating. Information overload, interface changes, and difficulty interpreting tone increased isolation rather than reducing it. PT OT SLP Systematic Review 2022 Cognitive impairment, anxiety and depression: a map of Cochrane evidence relevant to rehabilitation for people with post COVID-19 condition. European journal of physical and rehabilitation medicine Cochrane reviews show cognitive training, exercise, and memory rehabilitation can help thinking and mood in stroke, cancer, and dementia. Evidence quality is low to very low. These findings apply to other conditions, not directly to post-COVID-19. OT Protocol 2022 Development of a behavioural intervention for cognitive fatigability in multiple sclerosis: Protocol for a pilot and feasibility study. Frontiers in rehabilitation sciences This paper outlines a plan for an 8-week videoconference program targeting cognitive fatigability in multiple sclerosis. No patients were treated and no results are reported. It describes how researchers will test feasibility before running a larger trial. PT OT SLP Cohort 2022 Rehabilitation of Patients With Acute Ischemic Stroke Who Required Assistance Before Hospitalization Contributes to Improvement in Activities of Daily Living: A Nationwide Database Cohort Study. Archives of rehabilitation research and clinical translation Starting rehabilitation early and increasing daily duration improved daily living activities in stroke patients who needed help before admission. This benefit held for those with moderate disability, but early rehabilitation did not improve outcomes for the most severely disabled group. PT OT SLP RCT 2023 Optimized Home Rehabilitation Technology Reduces Upper Extremity Impairment Compared to a Conventional Home Exercise Program: A Randomized, Controlled, Single-Blind Trial in Subacute Stroke. Neurorehabilitation and neural repair Patients using a sensor-based home exercise system improved upper limb function more than those using paper instructions. The trial stopped early due to the pandemic with only 25 participants analyzed, so the result is promising but not yet definitive for routine care. PT OT SLP Qualitative 2023 Exemplary post-discharge stroke rehabilitation programs: A multiple case study. Clinical rehabilitation Exemplary post-discharge stroke care relies on three pillars: deep clinical knowledge, a respectful personalized relationship, and team commitment. Patients valued providers who treated them like friends, explained stroke impacts clearly, and offered flexible timing rather than rigid intensity. PT OT SLP Cohort 2023 The Moderating Role of Motivation in the Real-Time Associations of Fatigue, Cognitive Complaints, and Pain With Depressed Mood Among Stroke Survivors: An Ecological Momentary Assessment Study. Archives of physical medicine and rehabilitation For stroke survivors, feeling tired, in pain, or mentally slow was linked to feeling depressed in the moment. However, doing activities because they were personally meaningful, rather than out of pressure or obligation, weakened this link. Clinicians should support autonomous motivation. PT OT SLP Other 2022 Effect of exoskeleton-assisted Body Weight-Supported Treadmill Training on gait function for patients with chronic stroke: a scoping review. Journal of neuroengineering and rehabilitation Exoskeleton-assisted treadmill training improved walking in 10 of 11 studies, but it was not better than therapist-assisted training. The robot's value appears to be reducing physical burden on therapists while maintaining similar gait gains, not adding superior efficacy. PT OT SLP Systematic Review 2022 Dysphagia, dysphonia and olfactory disease: a map of Cochrane evidence relevant to rehabilitation for people with post COVID-19 condition. European journal of physical and rehabilitation medicine No Cochrane reviews exist for treating voice or smell loss in post-COVID. For swallowing, one review of stroke patients suggests behavioral strategies and TMS may help, but evidence quality is low and does not directly prove these treatments work for post-COVID patients. PT OT SLP Other 2022 Interaction of network and rehabilitation therapy parameters in defining recovery after stroke in a Bilateral Neural Network. Journal of neuroengineering and rehabilitation In a computer model of stroke, exploratory movements across a full workspace improved reaching accuracy more than repetitive stereotypic movements. This benefit occurred regardless of whether the healthy arm was restrained or used. The model suggests movement variety, not hand selection, drives recovery. PT OT Cohort 2022 Characteristics Associated With Perceived Underemployment Among Participants With Spinal Cord Injury. Archives of rehabilitation research and clinical translation Among employed people with spinal cord injury, 24.6% perceived themselves as underemployed. Higher earnings, health benefits, and being married were linked to lower odds of feeling underemployed, while working in sales or management/professional roles was linked to higher odds. PT OT Other 2023 Psychometric Properties of a New Measure of Upper Limb Performance in Post-Stroke Individuals: Trunk-Based Index of Performance. Neurorehabilitation and neural repair A new measure called IPt combines speed, accuracy, and trunk compensation in stroke reaching. It showed excellent stability over time and could distinguish mild from moderate-to-severe impairment, offering a way to track motor skill changes beyond standard clinical scores. PT OT SLP Cohort 2022 Successful Community Discharge Among Older Adults With Traumatic Brain Injury Admitted to Inpatient Rehabilitation Facilities. Archives of rehabilitation research and clinical translation Most older adults with traumatic brain injury return home after inpatient rehab. Those living alone before injury were less likely to succeed than those living with others. Higher motor and cognitive scores on admission predicted a better chance of going home. PT OT SLP Systematic Review 2022 Service Delivery Models for the Management of Pediatric and Adolescent Concussion: A Systematic Review. Archives of rehabilitation research and clinical translation Most concussion studies measure provider habits or costs, not patient recovery. Specialists deliver active exercise and multidisciplinary care, while generalists often lack standardized tools. Telemedicine improves access for rural families but cannot replace in-person neurologic exams. PT OT SLP Cohort 2022 Factors Associated With Time to Achieve Physical Functional Recovery in Patients With Severe Stroke After Inpatient Rehabilitation: A Retrospective Nationwide Cohort Study in Japan. Archives of rehabilitation research and clinical translation Only 40.4% of patients with severe stroke achieved physical independence by discharge. Younger age and higher initial motor scores predicted faster recovery. Patients who reached this goal received 10 more minutes of therapy per day than those who did not. PT OT Other 2022 A Scoping Review of the Characteristics of Activity-based Therapy Interventions Across the Continuum of Care for People Living With Spinal Cord Injury or Disease. Archives of rehabilitation research and clinical translation Across 140 studies, activity-based therapy for spinal cord injury varied widely, with most research done in labs. Intensity was rarely reported. Clinicians should not assume a standard protocol exists; document specific exercises, dosage, and technology used for each patient. PT OT Cohort 2022 Longitudinal Median Nerve Ultrasound Changes in Individuals With Spinal Cord Injury and an Age- and Sex-Matched Nondisabled Cohort. Archives of rehabilitation research and clinical translation One year after spinal cord injury, patients reported more carpal tunnel symptoms than age-matched controls. Ultrasound changes in the median nerve were subtle and mostly appeared in the nondominant hand, suggesting monitoring for symptoms rather than relying on imaging alone. ← Previous PAGE 28 OF 56 Next →
