Missed doses were 1.8%, and staff used safety huddles, dose checks and cold-chain controls. It describes operations, not proof the model works.

It highlights that root cause analysis is widely used but often inconsistent, relying too heavily on interviews. The authors suggest combining interviews with observation techniques and accountability matrices to better identify systemic failures.
