PT OT SLP RCT 2017 Improving Prospective Memory in Persons With Parkinson Disease: A Randomized Controlled Trial. Neurorehabilitation and neural repair Teaching people with Parkinson's to visualize specific 'when and what' cues improved memory for one-off tasks more than simple repetition. Both strategies helped, but the visualization method showed a stronger effect on difficult, non-repeated tasks. PT OT Other 2017 The added value of kinematic evaluation of the timed finger-to-nose test in persons post-stroke. Journal of neuroengineering and rehabilitation Persons post-stroke completed the finger-to-nose test slower, with less smooth, less straight and less accurate pointing and more trunk and shoulder-blade movement. Even when their time matched controls, these movement differences remained. PT OT Case-Control 2017 Kinesthetic deficits after perinatal stroke: robotic measurement in hemiparetic children. Journal of neuroengineering and rehabilitation Children with hemiparetic CP from perinatal stroke have measurable kinesthetic deficits. Arterial strokes caused more severe impairments than venous ones. Vision did not compensate for these deficits. Clinical sensory exams often missed the robotic deficits. OT RCT 2017 The effect of self-management education following mild stroke: an exploratory randomized controlled trial. Topics in stroke rehabilitation Self-management education after mild stroke produced some reported changes, but compared with control the benefit was minimal. In 71 randomized participants, it did not support adding this program. OT Cohort 2017 Rehospitalization in the First Year Following Veteran and Service Member TBI: A VA TBI Model Systems Study. The Journal of head trauma rehabilitation Veterans and Service Members after inpatient TBI rehabilitation were readmitted often: 164 of 401 had at least one hospitalization within a year, and inpatient rehabilitation was the leading reason. Longer post-traumatic amnesia and delayed command following were associated with rehospitalization. OT Pilot 2017 Immune biomarkers for the diagnosis of mild traumatic brain injury. NeuroRehabilitation In 28 people, blood immune-gene patterns separated all 9 TBI patients from all 19 controls, and white blood cell ratios changed over 48 hours. This was a small pilot, not a tested clinical triage test. PT OT SLP Pilot 2017 Development and assessment of a hand assist device: GRIPIT. Journal of neuroengineering and rehabilitation Four people with spinal cord injury wrote with GRIPIT, a manual glove that tightens a wire to form a tripod grasp. They reported better writing sensation, less fatigue and clearer letters than with a penholder or their own hand, but found it fiddly to put on. PT OT Systematic Review 2017 Interactive wearable systems for upper body rehabilitation: a systematic review. Journal of neuroengineering and rehabilitation Most included wearable upper-body rehab systems track motion and give visual feedback, but only three had clinical trials. Do not assume they improve movement or function; evidence is still mainly technical and usability. OT RCT 2017 The effects of repetitive transcranial magnetic stimulation (rTMS) on depression, visual perception, and activities of daily living in stroke patients. Journal of physical therapy science The rTMS group had lower depression scores and higher visual perception and ADL scores over 4 weeks, but the control group also improved by week 8. All 44 stroke patients with unilateral neglect also received conventional rehabilitation, so rTMS was not shown to work alone. PT OT Other 2017 Nonlinear mixed-effects model reveals a distinction between learning and performance in intensive reach training post-stroke. Journal of neuroengineering and rehabilitation During intensive reach training after stroke, movement slowing can hide learning. In 16 people, a method that separated learning from fatigue-like worsening predicted one-month reach gains, while total performance change did not. PT OT SLP Cohort 2017 Trajectories of Functional Change After Inpatient Rehabilitation for Traumatic Brain Injury. Archives of physical medicine and rehabilitation Most TBI patients improved after inpatient rehab, but 8% stayed low and gained little. Older age was linked to a lower motor group, while Black or Hispanic ethnicity and longer wait were linked to lower groups. This flags prognosis, not proof of treatment. PT OT Other 2017 Stroke Lesions in a Large Upper Limb Rehabilitation Trial Cohort Rarely Match Lesions in Common Preclinical Models. Neurorehabilitation and neural repair ICARE did not show a difference in its main motor outcome between rehabilitation groups. The analyzed strokes were small, and common cortical models captured 16% of participants; a subcortical model captured 33%. OT SLP RCT 2017 Problem-Solving After Traumatic Brain Injury in Adolescence: Associations With Functional Outcomes. Archives of physical medicine and rehabilitation Maladaptive problem-solving style was associated with worse everyday functioning and self-reported impairment in adolescents after TBI. Severe and moderate TBI did not differ on problem-solving measures. The total problem-solving score was higher despite lower rational problem-solving. PT OT RCT 2017 Rehabilitation of hand function after spinal cord injury using a novel handgrip device: a pilot study. Journal of neuroengineering and rehabilitation Grip strength and independence scores did not show clear improvement, but task accuracy rose from 9.01% to 21.7%. The weekly handgrip protocol was feasible in 17 chronic cervical SCI subjects. PT OT Case Report 2017 Overground vs. treadmill-based robotic gait training to improve seated balance in people with motor-complete spinal cord injury: a case report. Journal of neuroengineering and rehabilitation Overground robotic gait training (Ekso) engaged trunk muscles and improved seated balance in three people with motor-complete spinal cord injury. Treadmill-based training (Lokomat) did not engage trunk muscles or improve seated balance. OT Pilot 2017 Body-Machine Interface Enables People With Cervical Spinal Cord Injury to Control Devices With Available Body Movements: Proof of Concept. Neurorehabilitation and neural repair With practice, eight people with cervical spinal cord injury learned to move a computer cursor using remaining shoulder movements, becoming faster and more accurate in a small proof of concept study. PT OT Systematic Review 2017 Is robot-assisted therapy effective in upper extremity recovery in early stage stroke? -a systematic literature review. Journal of physical therapy science Robot-assisted therapy matched conventional therapy for upper limb recovery in early stroke, but most studies added robots to usual care rather than replacing it. The benefit of the robot itself remains unclear, though 3D devices may aid motor function. PT OT Other 2017 The effects of training using EMG biofeedback on stroke patients upper extremity functions. Journal of physical therapy science Adding EMG biofeedback to traditional stroke rehabilitation did not clearly improve upper-limb or independence scores beyond traditional rehabilitation alone in 30 chronic stroke outpatients. OT Other 2017 Quantification of task-dependent cortical activation evoked by robotic continuous wrist joint manipulation in chronic hemiparetic stroke. Journal of neuroengineering and rehabilitation After stroke, people with severe sensory impairment had reduced brain responses to robotic wrist movement. People with mild impairment showed reduced responses during active wrist tasks. The findings describe sensory pathway damage, not treatment benefit. PT OT Narrative Review 2017 Consideration of Dose and Timing When Applying Interventions After Stroke and Spinal Cord Injury. Journal of neurologic physical therapy : JNPT For upper limb stroke rehabilitation, more repetitions did not clearly improve function, and measured gains did not translate into daily use. Walking after stroke and incomplete spinal cord injury may respond to intensive therapy, but timing and inflammation matter. PT OT Pilot 2017 A Neuromuscular Electrical Stimulation (NMES) and robot hybrid system for multi-joint coordinated upper limb rehabilitation after stroke. Journal of neuroengineering and rehabilitation After 20 sessions with a muscle-signal-controlled robot and electrical stimulation device, 11 people after chronic stroke improved upper-limb function scores and tracking accuracy. No control group means the combined intervention cannot be attributed to one component. PT OT Narrative Review 2017 Activity-Based Therapy: From Basic Science to Clinical Application for Recovery After Spinal Cord Injury. Journal of neurologic physical therapy : JNPT Intense, task-specific movement below the spinal cord injury is the core of activity-based therapy. The review also describes a movement-quality scale and three children with chronic SCI reported to change following locomotor training. PT OT SLP Cohort 2017 Focused and divided attention abilities in the acute phase of recovery from moderate to severe traumatic brain injury. Brain injury People with moderate-to-severe TBI in acute recovery struggle to search visually when distracted, but can filter out irrelevant info if first given a spatial cue. They are also slower to confirm when a target is absent. Use location cues and allow extra time. OT Other 2017 Design and test of an automated version of the modified Jebsen test of hand function using Microsoft Kinect. Journal of neuroengineering and rehabilitation In 11 stroke patients, a Kinect camera timed the Modified Jebsen hand test and agreed with video review better than a therapist did. It may help repeat hand-function tests consistently, but does not show recovery. PT OT SLP Cohort 2017 A Short and Distinct Time Window for Recovery of Arm Motor Control Early After Stroke Revealed With a Global Measure of Trajectory Kinematics. Neurorehabilitation and neural repair Arm motor control recovery plateaued by 5 weeks post-stroke, while strength and clinical scores continued improving through one year. This suggests spontaneous biological recovery is largely complete early, whereas later gains likely reflect strength or compensation. OT Cohort 2017 Rehospitalization Over 10 Years Among Survivors of TBI: A National Institute on Disability, Independent Living, and Rehabilitation Research Traumatic Brain Injury Model Systems Study. The Journal of head trauma rehabilitation Rehospitalization after TBI rehabilitation discharge was common: 27.8% of participants at 1 year, 23.0% at 2 years, 22.1% at 5 years, and 23.4% at 10 years. Reasons moved toward general health and psychiatric causes. OT Cohort 2017 Anxiety and Posttraumatic Stress Disorder Among Medicare Beneficiaries After Traumatic Brain Injury. The Journal of head trauma rehabilitation After hospitalization for traumatic brain injury, older adults had more anxiety and PTSD diagnoses than before injury. Anxiety appeared in 17% and PTSD in 0.3%, with rates highest in the first months. OT Other 2017 The Minimal Clinically Important Difference for the Mayo-Portland Adaptability Inventory. The Journal of head trauma rehabilitation A 5-point rise on the MPAI-4 total score may mark the smallest meaningful improvement, and a 9-point rise a moderate improvement, in adults during post-inpatient brain injury rehabilitation. PT OT Other 2017 Inter-rater reliability of kinesthetic measurements with the KINARM robotic exoskeleton. Journal of neuroengineering and rehabilitation When different robot operators set up the same participant, scores for matching arm movement were similar. The test can be used consistently by different operators, but it did not test treatment effects or daily function. PT OT Other 2017 The Impact of Shoulder Abduction Loading on Volitional Hand Opening and Grasping in Chronic Hemiparetic Stroke. Neurorehabilitation and neural repair Lifting the affected arm against gravity worsened hand opening in moderate chronic stroke and made severely impaired participants close their hand instead of opening it. Controls were unaffected. Total maximal grasping force did not change, but involuntary grasping force rose with loading. PT OT Qualitative 2016 How Therapists Use Visualizations of Upper Limb Movement Information From Stroke Patients: A Qualitative Study With Simulated Information. JMIR rehabilitation and assistive technologies Therapists saw simulated arm-movement dashboards as useful for checking home activity, exercise, and neglect, engaging patients, and advocating for resources. They also said the data lacked context needed to tell whether movements were exercises, daily tasks, or compensations. PT OT Case-Control 2017 Abnormal EEG Responses to TMS During the Cortical Silent Period Are Associated With Hand Function in Chronic Stroke. Neurorehabilitation and neural repair In chronic stroke, brain stimulation with brain-wave recording during active thumb contraction showed abnormal brain responses and a longer pause in muscle activity than controls. Hand-function links were tentative and not confirmed. PT OT Meta-Analysis 2017 Markerless motion capture systems as training device in neurological rehabilitation: a systematic review of their use, application, target population and efficacy. Journal of neuroengineering and rehabilitation Most markerless motion capture rehab studies used Kinect in stroke, training upper or lower limbs. Patients improved on balance or arm tests, but few trials, low quality and varied protocols mean efficacy is not proven. PT OT SLP Systematic Review 2017 Needs of Stroke Survivors as Perceived by Their Caregivers: A Scoping Review. American journal of physical medicine & rehabilitation Caregivers most often reported environmental needs, especially support, services, safety and accessibility, after discharge. They also noted psychosocial, physical, cognitive and speech needs. This review maps those needs. It does not test whether meeting them improves outcomes. OT Qualitative 2017 Opportunities and barriers for successful return to work after acquired brain injury: A patient perspective. Work (Reading, Mass.) Ten people with acquired brain injury who had returned to work described individually adapted rehabilitation, balanced motivation, and understanding of cognitive and social needs as important. Barriers included fatigue, memory problems, low self-esteem, and unhelpful workplace attitudes. PT OT Other 2017 KAPS (kinematic assessment of passive stretch): a tool to assess elbow flexor and extensor spasticity after stroke using a robotic exoskeleton. Journal of neuroengineering and rehabilitation A robotic elbow stretch test found abnormal movement responses in people after stroke, including some without clinical spasticity. It may reveal resistance missed by Ashworth and Tardieu scores, but it is not yet a routine clinical tool. OT Other 2016 Tactile Perception for Stroke Induce Changes in Electroencephalography. Hong Kong journal of occupational therapy : HKJOT Active touch raised brain-wave activity more than passive touch in people with left-hemisphere stroke, especially right prefrontal and parietal areas. Right-hemisphere stroke showed no clear difference. This was a brain-wave lab finding, not a functional outcome. OT Qualitative 2017 Qualitative Experience of Sleep in Individuals With Spinal Cord Injury. OTJR : occupation, participation and health People with spinal cord injury described poor sleep linked to pain, positioning, pressure-ulcer care, bladder routines, medications, environment and anxiety. They also connected it to daytime fatigue and missed valued activities. PT OT Other 2017 The Arm Movement Detection (AMD) test: a fast robotic test of proprioceptive acuity in the arm. Journal of neuroengineering and rehabilitation A robotic test that took about twelve minutes separated stroke survivors with impaired arm position sense from those without. It also detected age-related changes in movement detection threshold, but it did not test treatment benefit. PT OT Cohort 2017 "Long-term stability of stimulating spiral nerve cuff electrodes on human peripheral nerves". Journal of neuroengineering and rehabilitation In people with implanted spiral nerve cuff electrodes, most stimulation contacts still produced motor or sensory responses, and stimulation settings changed little over 5 to 11 years. OT Other 2017 Inhibition of Parkinsonian tremor with cutaneous afferent evoked by transcutaneous electrical nerve stimulation. Journal of neuroengineering and rehabilitation In 8 people with Parkinson's disease, electrical stimulation on the back of the hand reduced resting tremor during stimulation by 61.56% on average, but tremor returned after stimulation and some trials worsened. This is a small lab finding, not a proven treatment. OT Other 2017 Assessment of unilateral neglect in stroke: Simplification and structuring of test items. The British journal of occupational therapy In stroke inpatients with known neglect, structured feeding, dressing, item location, and cancellation tasks showed little neglect, while naturalistic observation captured neglect. Simplifying and structuring tasks may let patients compensate, so use occupation-based observation. PT OT Cohort 2017 Prediction of prognosis of upper-extremity function following stroke-related paralysis using brain imaging. Journal of physical therapy science In 12 stroke patients with partly intact motor tract fibers, a diffusion MRI tract ratio was related to upper-limb motor and use scores at 3 months. Two cases did not follow this pattern, and all received physical, occupational, and speech therapy. OT Other 2017 Spanish Transcultural Adaptation and Validity of the Behavioral Inattention Test. Occupational therapy international Spanish stroke patients with neglect differed from those without neglect on every adapted BIT item. The test gave consistent scores and may help therapists assess everyday neglect problems. OT Other 2017 Test-Retest Reliability of a Measure of Independence in Everyday Activities: The ADL Profile. Occupational therapy international The ADL Profile produced stable scores when the same therapist retested 20 people with severe TBI. Three community tasks were less stable, so a better score on those tasks may not mean change. OT RCT 2017 Robotic Assistance for Training Finger Movement Using a Hebbian Model: A Randomized Controlled Trial. Neurorehabilitation and neural repair High robotic assistance did not improve the primary hand function outcome more than low assistance, but both groups improved. Higher assistance was more motivating and was linked to more benefit for more severely impaired patients with intact proprioception. OT SLP RCT 2017 Teen online problem solving for teens with traumatic brain injury: Rationale, methods, and preliminary feasibility of a teen only intervention. Rehabilitation psychology Teens and parents rated the overall online problem-solving program similarly, but parents in the teen-only group reported less benefit on several items. Teens completed a similar number of sessions with limited parent involvement as in the family version. OT Other 2017 Validity and Reliability of the Korean Version of the Utrecht Scale for Evaluation of Rehabilitation-Participation. Occupational therapy international The Korean Utrecht participation scale was most stable for stroke survivors' satisfaction with daily life. Its activity frequency and participation restriction scores were less steady over two weeks, so use them with caution. PT OT Other 2017 Cross-cultural adaptation and measurement properties of the Brazilian version of the Exercise Preference Questionnaire(stroke). Brazilian journal of physical therapy The Brazilian Portuguese stroke exercise preference questionnaire was usable and mostly consistent across repeat answers, but items about instructions and planning were less dependable. It can help ask patients what exercise settings they prefer, not show an exercise program works. PT OT Other 2017 Lateral Perturbation-Induced Stepping: Strategies and Predictors in Persons Poststroke. Journal of neurologic physical therapy : JNPT When pushed sideways, people with chronic stroke rarely stepped with their weak leg. They mostly used their strong leg and took multiple steps. Poor sensation in the weak foot made them avoid using it. This suggests therapy should target stepping with the affected side. ← Previous PAGE 5 OF 56 Next →
