PT OT RCT 2019 Results From a Randomized Controlled Trial to Address Balance Deficits After Traumatic Brain Injury. Archives of physical medicine and rehabilitation Virtual reality balance games were not better than traditional home exercise programs for people with chronic traumatic brain injury. Both groups improved similarly over 24 weeks, so neither approach is superior for treating balance deficits. PT OT SLP RCT 2019 Elements virtual rehabilitation improves motor, cognitive, and functional outcomes in adult stroke: evidence from a randomized controlled pilot study. Journal of neuroengineering and rehabilitation In 21 sub-acute stroke inpatients, adding Elements tabletop VR to usual occupational and physiotherapy produced larger gains on the Box and Blocks test, cognition screening, and self-rated daily function than usual therapy alone. Gains remained at one month. Small unblinded pilot. PT OT Case-Control 2019 Stepping characteristics during externally induced lateral reactive and voluntary steps in chronic stroke. Gait & posture Stroke survivors initiated voluntary steps slower than controls but reacted to perturbations just as fast. They used smaller, medial steps to recover balance, likely because weak hip muscles make the more stable lateral steps too difficult to execute quickly. PT OT Other 2019 Quality of life after spinal cord injury in Thai individuals: A mixed-methods study. Hong Kong physiotherapy journal : official publication of the Hong Kong Physiotherapy Association Limited = Wu li chih liao Thai people with spinal cord injury had much lower physical health than Thai norms. Their vitality and social functioning were not lower. Interviews tied quality of life to paid work, personal assistants, accessible environments, stigma and self-advocacy. OT SLP Other 2019 Characterizing computer-mediated communication, friendship, and social participation in adults with traumatic brain injury. Brain injury Adults with TBI contacted friends by face-to-face, phone, email, and social media at similar rates to peers, but used texting less often as their main method. They also had fewer friends and lower social participation. OT SLP Survey 2019 Flourishing in children with autism spectrum disorders. Autism research : official journal of the International Society for Autism Research Children with autism had lower parent-rated social competence and behavioral control than peers, but school motivation was similar. This helps therapists target social and regulation goals, but it does not show treatment works. PT OT SLP Other 2019 The influence of posture duration on hand tremor during tasks with attention-distraction in persons with Parkinson's disease. Journal of neuroengineering and rehabilitation Hand tremor intensity in Parkinson's disease increases significantly over a 30-second task, while it stays constant in healthy controls. Clinicians should record tremor for at least 30 seconds to capture this time-dependent amplification. PT OT Cohort 2019 The items and level contributing to improve in less than 50 motor functional independence measure upon admission in the stroke recovery rehabilitation ward. Journal of physical therapy science In stroke rehab patients starting below 50 independence points, gains were linked to bathing, lower-body dressing, bladder management and stairs. These are associations from routine records, not a tested treatment effect. PT OT RCT 2019 Long-Dose Intensive Therapy Is Necessary for Strong, Clinically Significant, Upper Limb Functional Gains and Retained Gains in Severe/Moderate Chronic Stroke. Neurorehabilitation and neural repair Severe/moderate chronic stroke survivors who received 300 hours of intensive upper-limb therapy improved coordination and function. Gains remained at 3 months, and functional task performance improved further. PT OT Other 2019 Chronicity of Stroke Does Not Affect Outcomes of Somatosensory Stimulation Paired With Task-Oriented Motor Training: A Secondary Analysis of a Randomized Controlled Trial. Archives of rehabilitation research and clinical translation Time since stroke did not predict how much arm movement improved after paired sensory stimulation and task training in adults 3 to 12 months after stroke. Both groups improved, and early versus later enrollment did not change the response. PT OT SLP RCT 2019 Walking to your right music: a randomized controlled trial on the novel use of treadmill plus music in Parkinson's disease. Journal of neuroengineering and rehabilitation Adding rhythmic music to treadmill training improved gait quality, balance, and walking function more than treadmill training alone in people with Parkinson's. Both groups improved, but the music group showed greater gains in the primary gait assessment. PT OT Cohort 2019 Differential Poststroke Motor Recovery in an Arm Versus Hand Muscle in the Absence of Motor Evoked Potentials. Neurorehabilitation and neural repair After stroke, absent motor-evoked potentials did not always prevent elbow flexor contraction or strength recovery, but finger muscle contraction, strength, and standardized arm-hand movement scores remained impaired. PT OT Other 2019 Trading Symmetry for Energy Cost During Walking in Healthy Adults and Persons Poststroke. Neurorehabilitation and neural repair People walk asymmetrically if it saves energy. Stroke survivors changed gait symmetry when a controller made new patterns less costly, but they did not spontaneously prefer symmetric walking. This suggests rehabilitation must make desired movements energetically beneficial to be adopted. PT OT Other 2019 What is the impact of user affect on motor learning in virtual environments after stroke? A scoping review. Journal of neuroengineering and rehabilitation Virtual reality and active video gaming stroke studies often mention motivation, enjoyment, engagement, immersion, or presence, but only 50 of 155 measured them. No study tested whether these experiences were connected to motor learning, so their impact remains unknown. PT OT SLP Other 2019 Sleep problems contribute to post-concussive symptoms in service members with a history of mild traumatic brain injury without posttraumatic stress disorder or major depressive disorder. NeuroRehabilitation In service members with remote mild TBI, sleep problems were linked to post-concussive symptoms mainly when PTSD or depression was absent. Daytime dysfunction and nighttime disruptions were associated with somatosensory symptoms, not cognitive or balance symptoms. OT SLP RCT 2019 Neurocognitive Driving Rehabilitation in Virtual Environments (NeuroDRIVE): A pilot clinical trial for chronic traumatic brain injury. NeuroRehabilitation Driving simulator performance did not improve more than the wait list after NeuroDRIVE. Working memory, visual search, and self-reported physical functioning did improve more in the treated group, in a small chronic TBI pilot. PT OT Cohort 2019 End-point kinematics using virtual reality explaining upper limb impairment and activity capacity in stroke. Journal of neuroengineering and rehabilitation Movement speed and smoothness from a virtual pointing task tracked some upper limb impairment and activity capacity after stroke, but only partly matched standard clinical scores. Use them as added information, not as a replacement. PT OT RCT 2019 Combining Fluoxetine and rTMS in Poststroke Motor Recovery: A Placebo-Controlled Double-Blind Randomized Phase 2 Clinical Trial. Neurorehabilitation and neural repair Combining low-frequency rTMS with fluoxetine improved hand function speed significantly more than either fluoxetine or placebo alone. However, the fluoxetine-only group actually performed worse than the placebo group. The benefit likely came from rTMS overcoming potential negative effects of the drug, not from synergy. OT Other 2019 Egocentric video: a new tool for capturing hand use of individuals with spinal cord injury at home. Journal of neuroengineering and rehabilitation A head-mounted video system detected hand-object interactions in nine people with cervical spinal cord injury. Its interaction counts and durations tracked human observer labels moderately well, but it was tested in a simulated lab, not real homes. PT OT Other 2019 The "Beam-Me-In Strategy" - remote haptic therapist-patient interaction with two exoskeletons for stroke therapy. Journal of neuroengineering and rehabilitation Fifteen therapists used two linked exoskeleton robots to feel simulated stroke impairments in their own arms. They reliably distinguished healthy from severely affected movement patterns. This haptic feedback let them assess range of motion and muscle tone remotely, though accuracy varied by task. PT OT Survey 2019 Self-Efficacy, Self-Regulation, Social Support, and Outcomes Expectations for Daily Physical Activity in Adults with Chronic Stroke: A Descriptive, Exploratory Study. Occupational therapy in health care Ambulatory chronic stroke survivors reported high confidence and positive expectations for daily physical activity, but low use of self-regulation and social support. These patterns point to planning, routines, and social help as possible areas for support. OT Other 2019 Effects of the Computer Desk Level on the Musculoskeletal Discomfort of Neck and Upper Extremities and EMG Activities in Patients with Spinal Cord Injuries. Occupational therapy international In 12 wheelchair users with spinal cord injury, self-rated neck and arm discomfort changed with desk height by injury level: higher desk lowered discomfort for C6, lower desk lowered discomfort for T2-T6. EMG muscle activity did not change. OT Other 2019 Upper Extremity Function Assessment Using a Glove Orthosis and Virtual Reality System. OTJR : occupation, participation and health A sensor-equipped glove tracking hand movements during virtual tasks correlates strongly with the Wolf Motor Function Test in stroke survivors. This suggests the software can validly monitor upper limb motor status during therapy without requiring separate clinical assessments. PT OT RCT 2019 Forced, Not Voluntary, Aerobic Exercise Enhances Motor Recovery in Persons With Chronic Stroke. Neurorehabilitation and neural repair In 40 adults with chronic stroke, forced aerobic cycling plus upper limb task practice improved Fugl-Meyer scores by 11 points, versus 6 with voluntary cycling plus task practice and 9 with education plus task practice. The forced group improved most. OT SLP Cohort 2019 Can the stroke impact scale 3.0 detect cognitive impairments in patients with a recent stroke? Journal of physical therapy science After stroke rehab, 41 older survivors using the Stroke Impact Scale reported trouble with memory, concentration and everyday problem solving, and many reported low mood items. The scale flagged concerns, but the study did not prove it detects cognitive impairment. OT Other 2019 Canadian Occupational Performance Measure Supported by Talking Mats: An Evaluation of the Clinical Utility. Occupational therapy international For six clients with cognitive and communication problems after brain injury, adding Talking Mats to the COPM helped most identify and rate daily activity problems, especially early steps. One client was less engaged and reflective. Use it selectively. OT SLP Cohort 2019 Behavioral Health Service Utilization and Unmet Need After Traumatic Brain Injury in Childhood. Journal of developmental and behavioral pediatrics : JDBP Years after injury, children with traumatic brain injury had higher rates of behavioral health need than those with other injuries, but service use did not differ. Across all groups, 77% of those needing care were not receiving it. Race predicted access, with white children having higher unmet need. PT OT RCT 2019 Intensive virtual reality and robotic based upper limb training compared to usual care, and associated cortical reorganization, in the acute and early sub-acute periods post-stroke: a feasibility study. Journal of neuroengineering and rehabilitation In people with recent stroke, adding intensive virtual reality and robotic arm training to usual inpatient therapy was associated with larger 6-month gains in arm impairment and wrist movement, but not in task performance or pinch. OT Other 2019 Development and Validation of the Occupational Therapy Engagement Scale for Patients with Stroke. Occupational therapy international A 12-item scale allowed occupational therapists to rate engagement in hospitalized subacute stroke patients. Higher scores meant more engagement, and scores were modestly related to daily-living performance two months after discharge. PT OT RCT 2019 Effects of Whole-Body Vibration on Upper Extremity Function and Grip Strength in Patients with Subacute Stroke: A Randomised Single-Blind Controlled Trial. Occupational therapy international Adding whole-body vibration before task-oriented training improved upper extremity function and grip strength more than upper- and lower-cycle training, but both groups also had task-oriented and conventional therapy, so vibration alone is not proven. OT Other 2019 Reliability and validity of the Japanese version of the ADL-focused Occupation-based Neurobehavioural Evaluation (A-ONE J): Applying Rasch analysis methods. Hong Kong journal of occupational therapy : HKJOT The Japanese ADL-focused Neurobehavioural Evaluation works as a reliable outcome measure for stroke patients when its 20 activity items are scored on a simplified three-category scale. Therapists should use this revised scoring method to track changes in functional independence. OT SLP Cohort 2019 Responsiveness, Minimal Clinically Important Difference, and Validity of the MoCA in Stroke Rehabilitation. Occupational therapy international MoCA scores rose significantly in 65 stroke survivors after task-oriented therapy. The smallest clinically important change was estimated at 1.22 or 2.15 points, depending on method. Relationships with some stroke impact domains were fair to good. PT OT Systematic Review 2019 Need for mechanically and ergonomically enhanced tremor-suppression orthoses for the upper limb: a systematic review. Journal of neuroengineering and rehabilitation Existing upper-limb tremor orthoses suppress tremor by 63% on average in patients, matching medication, but are bulky and heavy. They add about 20% weight to the arm, causing fatigue and rejection. Current designs fail ergonomics despite decent mechanical suppression. PT OT Cohort 2019 Bimanual coordination deficits in hands following stroke and their relationship with motor and functional performance. Journal of neuroengineering and rehabilitation Stroke patients showed unstable grip-force transfer to the paretic hand, with sudden force rebounds. Shorter transfer times from the paretic hand correlated with better motor and functional scores, suggesting this specific timing metric may track recovery progress. PT OT RCT 2019 Perspectives of people with spinal cord injury learning to walk using a powered exoskeleton. Journal of neuroengineering and rehabilitation People with spinal cord injury described ReWalk training as helping them stand, walk, and do everyday things like peers. They reported bowel, spasticity, and pain changes, valued purpose and social contact, and said it was not ready for home use. PT OT Cohort 2019 Reweighting of the sensory inputs for postural control in patients with cervical spondylotic myelopathy after surgery. Journal of neuroengineering and rehabilitation Surgery improved walking scores but did not normalize balance. Patients with cervical myelopathy still relied less on leg sensation and more on vision and inner-ear balance than healthy adults, even six months after decompression. They remain at risk for falls and need specific balance retraining. OT Other 2019 Spatial navigation ability and gaze switching in older drivers: A driving simulator study. Hong Kong journal of occupational therapy : HKJOT Older drivers had lower scores on a spatial navigation test and different gaze patterns than young drivers in a simulator. In older drivers, lower Card-Placing scores went with longer gaze time. PT OT Case Report 2019 Myoelectric signal from below the level of spinal cord injury as a command source for an implanted upper extremity neuroprosthesis - a case report. Journal of neuroengineering and rehabilitation In a person with C6 spinal cord injury, an electrical signal from a foot muscle below the injury controlled an implanted hand grasp device. On a grasp-release object test, success matched his usual shoulder control, but this was a single case. OT Qualitative 2019 A cognitive occupation-based programme for people with multiple sclerosis: A new occupational therapy cognitive rehabilitation intervention. Hong Kong journal of occupational therapy : HKJOT People with MS and occupational therapists liked a proposed group programme for MS cognition, calling it a good fit. They said start it soon after diagnosis, but distance, caseloads and small group numbers could block delivery. PT OT SLP RCT 2020 Frontal lesions predict response to prism adaptation treatment in spatial neglect: A randomised controlled study. Neuropsychological rehabilitation Prism adaptation improved daily functional neglect only in stroke patients with frontal lesions. Those without frontal lesions saw no benefit over standard care. Paper-and-pencil tests missed this difference entirely. Use functional observation to guide treatment selection. OT RCT 2020 A qualitative analysis of pressure injury development among medically underserved adults with spinal cord injury. Disability and rehabilitation 25 adults with spinal cord injury developed medically serious pressure injuries during a prevention program, and notes identified themes including basic wound care knowledge gaps, equipment shortfalls, comorbidities, bed rest non-adherence, inactivity, or circumstances beyond the program. OT RCT 2020 Dissociable effects of attention vs working memory training on cognitive performance and everyday functioning following fronto-parietal strokes. Neuropsychological rehabilitation Compared with waiting, home computer training with weekly phone support reduced spatial bias after selective attention training, improved a closely related memory task after working memory training, and reduced reported everyday cognitive difficulty. PT OT Case Report 2020 Improved Mobility, Cognition, and Disease Severity in Corticobasal Degeneration of an African American Man After 12 Weeks of Adapted Tango: A Case Study. American journal of physical medicine & rehabilitation Short-term gains in mobility, cognition, and disease severity were seen after adapted tango in a man with corticobasal degeneration, but many gains faded by follow-up and depression worsened. PT OT SLP Cohort 2019 Risk Markers for Not Returning to Work Among Patients with Acquired Brain Injury: A Population-Based Register Study. Journal of occupational rehabilitation One year after brain injury, women, people born outside Sweden, lower education, aphasia, long hospital stays, mobility or self-care problems, pain, anxiety/depression, and lower satisfaction with rehabilitation process or planning were at higher risk of not returning to work. PT OT Cohort 2019 Longitudinal Effects of Medical Comorbidities on Functional Outcome and Life Satisfaction After Traumatic Brain Injury: An Individual Growth Curve Analysis of NIDILRR Traumatic Brain Injury Model System Data. The Journal of head trauma rehabilitation After moderate-severe brain injury, motor and cognitive independence rose for years, then fell by 10 years. Life satisfaction stayed stable. Anxiety, depression, PTSD, diabetes, and other comorbidities were associated with different function or life-satisfaction trajectories. These are associations, not proven causes. OT Cohort 2019 Parenting Behaviors after Moderate - Severe Traumatic Injury in Early Childhood. Developmental neurorehabilitation Across broad parenting styles, injury type did not predict change. Parents of children with severe or moderate TBI reported declining warmth and involvement over years, while permissive and authoritarian behaviors fell in all groups. PT OT SLP Survey 2019 Rehabilitation Practices in Patients With Moderate and Severe Traumatic Brain Injury. The Journal of head trauma rehabilitation Most ICU clinicians caring for moderate and severe TBI supported starting rehabilitation, but timing varied. They usually waited for extubation, normal intracranial pressure and hemodynamic stability, and cited staff and resource limits as barriers. PT OT SLP Cohort 2020 Functional Outcome Trajectories Following Inpatient Rehabilitation for TBI in the United States: A NIDILRR TBIMS and CDC Interagency Collaboration. The Journal of head trauma rehabilitation On average, survivors of complicated mild to severe TBI improved between 1 and 2 years after injury, then declined by 5 years, though many kept improvements. Age, insurance, discharge function and rehabilitation stay shaped different long-term disability and supervision paths. OT Cohort 2020 Parenting styles as a predictor of long-term psychosocial outcomes after traumatic brain injury (TBI) in early childhood. Disability and rehabilitation Years after early severe brain injury, children had worse behaviour, social skills, and everyday planning and self-control than children with bone injuries. Later parenting mattered: permissive parenting linked to worse outcomes, authoritative to better social and planning skills. OT Pilot 2019 Feasibility Study of Problem-Solving Training for Care Partners of Adults With Traumatic Brain Injury, Spinal Cord Injury, Burn Injury, or Stroke During the Inpatient Hospital Stay. Archives of rehabilitation research and clinical translation Care partners who completed problem-solving training during acute hospital stay reported high satisfaction and confidence after 2.6 sessions, but only 12 of 39 consented and few used app boosters. ← Previous PAGE 11 OF 56 Next →
