A Case of Acne Fulminans.

Clare Cardo McKegney, Deanna Schneider

PMID 35864020

WHAT IT FOUND

One adolescent developed sudden severe ulcerative acne with fever and joint pain after COVID-19.

A face-to-face exam found widespread lesions, leading to acne fulminans diagnosis and treatment with prednisone and isotretinoin.

Key findings

01A 17-year-old male presented with fatigue, low-grade fever, joint pain, and severely painful ulcerative hemorrhagic nodules with pustular-like lesions on the face, chest, and back.

02The diagnosis was acne fulminans after systemic symptoms, hemorrhagic ulcerative lesions, and a wound culture positive for Propionibacterium acnes with multiple organisms.

03He was prescribed oral prednisone and oral isotretinoin, and after treatment his inflammatory acne resolved.

What it does not show

A case report cannot show that acne fulminans follows COVID-19 or that prednisone and isotretinoin caused resolution. The paper says more data are needed from patients infected with COVID-19 to determine the true relationship between these conditions. Larger randomized controlled trials evaluating treatment of acne fulminans are lacking.

The easy way to misread this

Do not read this case as proof that COVID-19 caused acne fulminans or that prednisone and isotretinoin worked. The paper describes a patient and says more data are needed to determine the relationship with COVID-19.
