WHAT IT FOUND

Postpartum screening beyond depression, especially anxiety and PTSD tools, can find cases missed by depression screening alone.

But most studies did not show that screening led to more treatment or better symptoms, so referral pathways must be ready.

Key findings

01Anxiety screeners showed modest to excellent accuracy, and in studies comparing anxiety with depression-only screening, an additional 5-15% of people screened positive who would not have been found by depression screening alone.

02In studies without a comparison group, 14-47% of individuals screened for anxiety sought mental health care, and 35.1% of people positive for one or more ongoing disorders received a referral in one computerized screening study.

03The review could not pool results: no screening-outcome combination had enough studies for meta-analysis, and 97.9% of included studies lacked an unscreened comparison group while 97.8% had no control for confounding.

What it does not show

The search string was English only, so relevant articles in other languages may have been missed. Most included studies had no unscreened comparison group, so changes after screening cannot be confidently attributed to screening. The review could not combine results because too few studies used the same screening tool and outcome. Results could not be compared by race, ethnicity, or language because few studies reported them separately. No included study met the review's criteria for strong global quality.

The easy way to misread this

Do not conclude that postpartum screening beyond depression improves referral, treatment, or symptoms. Most studies lacked an unscreened comparison group, and no screening-outcome combination had enough studies to test effectiveness.
