Jiwon Baek, Hyein Kim, Seongae Kwon and 2 others

PMID 40129112

WHAT IT FOUND

Average 28-day suicidal ideation did not differ between high- and low-risk outpatients.

Daily depression, anxiety and stress were associated with higher momentary suicidal ideation, especially depression, and high-risk showed flatter daily patterns.

Key findings

01Average 28-day suicidal ideation and depression did not differ between high- and low-risk groups, while high-risk participants had higher anxiety and lower stress.

02Momentary depression, anxiety and stress were each positively associated with momentary suicidal ideation in both risk groups.

03High-risk participants showed flatter 28-day patterns of suicidal ideation, depression, anxiety and stress than low-risk participants.

What it does not show

The study was conducted during the COVID-19 pandemic, and the planned sample of 128 was reduced to 50, so the findings come from a small group in an unusual period. Only 10 low-risk participants were analysed, so comparisons with the 40 high-risk participants are limited. Suicide attempt history was self-reported and not verified in medical records, so risk-group classification could be wrong. Participants were South Korean psychiatric outpatients with recent suicidal ideation, so the results may not apply to other countries, settings or people without recent suicidal thoughts. The study measured internal mood and stress but did not capture external stressors or factors such as living arrangement, marital status and occupation, so it cannot explain all suicide risk. EMA completion was 76.1%, and some participants gave fewer than three responses on a day, so missing reports may affect the daily patterns. The number of analysed EMA responses is inconsistent between the text and the tables, which makes the data handling harder to check.

Declared interests

The authors declared no conflicts of interest. Funding came from the College of Nursing at Yonsei University and from Korean government research funds, including the National Research Foundation of Korea funded by the Ministry of Education and the Ministry of Science and ICT.

The easy way to misread this

Do not conclude that ecological momentary assessment or remote monitoring prevents suicide. This was an observational study in 50 outpatients, and it did not compare monitoring with usual care or test an intervention.
