PT OT Other 2017 Does the Finger-to-Nose Test measure upper limb coordination in chronic stroke? Journal of neuroengineering and rehabilitation In chronic stroke, finger-to-nose test time tracked shoulder-elbow timing, went with arm impairment and activity, and a 10.6-second cut-off separated mild from moderate-to-severe impairment. PT RCT 2017 The effects of Pilates exercise on cardiopulmonary function in the chronic stroke patients: a randomized controlled trials. Journal of physical therapy science Adding Pilates to usual stroke rehab improved resting heart rate, oxygen uptake and Timed Up and Go more than control in 20 chronic stroke patients, but both groups changed and the study was small. PT OT Case Report 2017 Improved functional independence measure facilitates return to home after paralyzed upper-limb training: a case report. Journal of physical therapy science The FIM score rose from 71 to 105 in one stroke patient who received occupational therapy, affected upper-limb training, physiotherapy, and a fixed walker with knee support over four months. She returned home. This is one person, not proof the package works. PT Other 2017 In-lab versus at-home activity recognition in ambulatory subjects with incomplete spinal cord injury. Journal of neuroengineering and rehabilitation In incomplete spinal cord injury, a waist accelerometer recognized instructed home activities much better when trained on at-home movement data (85.9%) than when trained only on lab data (54.6%). PT OT SLP RCT 2017 Improving Prospective Memory in Persons With Parkinson Disease: A Randomized Controlled Trial. Neurorehabilitation and neural repair Teaching people with Parkinson's to visualize specific 'when and what' cues improved memory for one-off tasks more than simple repetition. Both strategies helped, but the visualization method showed a stronger effect on difficult, non-repeated tasks. PT Cohort 2017 Effect of Cue Timing and Modality on Gait Initiation in Parkinson Disease With Freezing of Gait. Archives of physical medicine and rehabilitation External cues, especially predictable sound or light cues, improved preparatory weight shifts before stepping and reduced missing preparatory movements in people with Parkinson disease with or without freezing. Ankle vibration cues helped less and were unreliable. PT RCT 2017 Effects of task-oriented circuit training on balance and gait ability in subacute stroke patients: a randomized controlled trial. Journal of physical therapy science Task-oriented circuit training improved walking endurance more than conventional therapy in subacute stroke patients. The circuit group walked 336.7 meters after 6 minutes, compared to 202.8 meters in the control group. Both groups improved balance and speed, but only endurance differed significantly. PT RCT 2017 The immediate effect of diaphragm taping with breathing exercise on muscle tone and stiffness of respiratory muscles and SpO2 in stroke patient. Journal of physical therapy science Diaphragm taping added to breathing exercise showed no significant difference from breathing exercise alone for immediate respiratory muscle tone, stiffness, or blood oxygen saturation in groups of 14 stroke patients. PT OT Other 2017 The added value of kinematic evaluation of the timed finger-to-nose test in persons post-stroke. Journal of neuroengineering and rehabilitation Persons post-stroke completed the finger-to-nose test slower, with less smooth, less straight and less accurate pointing and more trunk and shoulder-blade movement. Even when their time matched controls, these movement differences remained. PT OT Case-Control 2017 Kinesthetic deficits after perinatal stroke: robotic measurement in hemiparetic children. Journal of neuroengineering and rehabilitation Children with hemiparetic CP from perinatal stroke have measurable kinesthetic deficits. Arterial strokes caused more severe impairments than venous ones. Vision did not compensate for these deficits. Clinical sensory exams often missed the robotic deficits. PT Systematic Review 2017 A systematic review of mechanisms of gait speed change post-stroke. Part 2: exercise capacity, muscle activation, kinetics, and kinematics. Topics in stroke rehabilitation No single laboratory measure consistently explained why stroke survivors walked faster after physical therapy. Across 25 studies, changes in oxygen use, muscle activity, force, or joint motion were inconsistent, so gait speed gains may come from multiple pathways. PT Other 2017 Anticipatory Postural Adjustment During Self-Initiated, Cued, and Compensatory Stepping in Healthy Older Adults and Patients With Parkinson Disease. Archives of physical medicine and rehabilitation In healthy adults, cued stepping increased postural preparation before stepping, while a balance recovery step decreased it. In mild Parkinson disease, the change across these conditions was less pronounced than in healthy adults, and levodopa did not change step preparation. PT OT SLP Pilot 2017 Development and assessment of a hand assist device: GRIPIT. Journal of neuroengineering and rehabilitation Four people with spinal cord injury wrote with GRIPIT, a manual glove that tightens a wire to form a tripod grasp. They reported better writing sensation, less fatigue and clearer letters than with a penholder or their own hand, but found it fiddly to put on. PT RCT 2017 Walking in fully immersive virtual environments: an evaluation of potential adverse effects in older adults and individuals with Parkinson's disease. Journal of neuroengineering and rehabilitation Twenty minutes of treadmill walking in an immersive headset did not increase simulator sickness or worsen balance in healthy older adults or people with mild to moderate Parkinson's disease. A few people showed symptoms, so monitor tolerance before using it. PT RCT 2017 Does Nordic Walking restore the temporal organization of gait variability in Parkinson's disease? Journal of neuroengineering and rehabilitation After three one-hour technique sessions, walking with Nordic poles made stride timing less random and more like healthy controls during a short test. It lengthened steps without changing speed. This small immediate effect is not proof of long-term benefit. PT OT Systematic Review 2017 Interactive wearable systems for upper body rehabilitation: a systematic review. Journal of neuroengineering and rehabilitation Most included wearable upper-body rehab systems track motion and give visual feedback, but only three had clinical trials. Do not assume they improve movement or function; evidence is still mainly technical and usability. PT Other 2017 Validity of the Instrumented Push and Release Test to Quantify Postural Responses in Persons With Multiple Sclerosis. Archives of physical medicine and rehabilitation Wireless sensors on the lower back and feet closely matched laboratory motion capture and force plate measurements during Push and Release balance steps in people with MS and healthy controls. People with MS needed more steps and longer time to regain stability. PT Meta-Analysis 2017 Robot-assisted gait training (Lokomat) improves walking function and activity in people with spinal cord injury: a systematic review. Journal of neuroengineering and rehabilitation In incomplete spinal cord injury, walking speed did not improve with Lokomat training compared with overground training in people assessed within six months, but walking distance did. In chronic injury, speed improved versus no intervention, while distance did not. PT RCT 2017 Immediate effects of ankle eversion taping on dynamic and static balance of chronic stroke patients with foot drop. Journal of physical therapy science For people with chronic stroke and foot drop, ankle eversion taping immediately improved balance compared with placebo taping or no taping. The effect was only immediate. PT Other 2017 Use of Nintendo Wii Balance Board for posturographic analysis of Multiple Sclerosis patients with minimal balance impairment. Journal of neuroengineering and rehabilitation A Wii Balance Board showed the same balance differences between people with MS and healthy controls as a laboratory force plate, but it overestimated absolute sway-path scores. Use it for comparisons with the same board, not exact scores. PT OT Other 2017 Nonlinear mixed-effects model reveals a distinction between learning and performance in intensive reach training post-stroke. Journal of neuroengineering and rehabilitation During intensive reach training after stroke, movement slowing can hide learning. In 16 people, a method that separated learning from fatigue-like worsening predicted one-month reach gains, while total performance change did not. PT OT SLP Cohort 2017 Trajectories of Functional Change After Inpatient Rehabilitation for Traumatic Brain Injury. Archives of physical medicine and rehabilitation Most TBI patients improved after inpatient rehab, but 8% stayed low and gained little. Older age was linked to a lower motor group, while Black or Hispanic ethnicity and longer wait were linked to lower groups. This flags prognosis, not proof of treatment. PT OT Other 2017 Stroke Lesions in a Large Upper Limb Rehabilitation Trial Cohort Rarely Match Lesions in Common Preclinical Models. Neurorehabilitation and neural repair ICARE did not show a difference in its main motor outcome between rehabilitation groups. The analyzed strokes were small, and common cortical models captured 16% of participants; a subcortical model captured 33%. PT OT RCT 2017 Rehabilitation of hand function after spinal cord injury using a novel handgrip device: a pilot study. Journal of neuroengineering and rehabilitation Grip strength and independence scores did not show clear improvement, but task accuracy rose from 9.01% to 21.7%. The weekly handgrip protocol was feasible in 17 chronic cervical SCI subjects. PT OT Case Report 2017 Overground vs. treadmill-based robotic gait training to improve seated balance in people with motor-complete spinal cord injury: a case report. Journal of neuroengineering and rehabilitation Overground robotic gait training (Ekso) engaged trunk muscles and improved seated balance in three people with motor-complete spinal cord injury. Treadmill-based training (Lokomat) did not engage trunk muscles or improve seated balance. PT RCT 2017 Auto detection and segmentation of daily living activities during a Timed Up and Go task in people with Parkinson's disease using multiple inertial sensors. Journal of neuroengineering and rehabilitation After adjustments for Parkinson's movement patterns, wearable sensor algorithms detected standing, sitting, walking and turning correctly in all 432 tested activity instances during a Timed Up and Go task in 12 people with early disease. PT Other 2017 2-dimensional analysis of low limb taping methods on ambulation for stroke patients. Journal of physical therapy science Stroke patients who had Kinesio or McConnell taping plus PNF had longer affected-side stride length than those who had PNF alone. Taping was added to therapy, so its separate effect cannot be known. PT OT Systematic Review 2017 Is robot-assisted therapy effective in upper extremity recovery in early stage stroke? -a systematic literature review. Journal of physical therapy science Robot-assisted therapy matched conventional therapy for upper limb recovery in early stroke, but most studies added robots to usual care rather than replacing it. The benefit of the robot itself remains unclear, though 3D devices may aid motor function. PT OT SLP Narrative Review 2017 Virtual Reality and Serious Games in Neurorehabilitation of Children and Adults: Prevention, Plasticity, and Participation. Pediatric physical therapy : the official publication of the Section on Pediatrics of the American Physical Therapy Association Virtual reality and serious games improve balance and motor function in children and adults with neurological conditions, but evidence for participation and fitness gains is mixed. Dose and system type matter, yet clinical adoption is hindered by lack of training and clear guidelines. PT OT Other 2017 The effects of training using EMG biofeedback on stroke patients upper extremity functions. Journal of physical therapy science Adding EMG biofeedback to traditional stroke rehabilitation did not clearly improve upper-limb or independence scores beyond traditional rehabilitation alone in 30 chronic stroke outpatients. PT Other 2017 Differences in muscle activity and temporal step parameters between Lokomat guided walking and treadmill walking in post-stroke hemiparetic patients and healthy walkers. Journal of neuroengineering and rehabilitation In 10 post-stroke patients, Lokomat guided walking at 50% guidance produced lower muscle activity than treadmill walking but made step timing more symmetrical. This was a lab comparison, not evidence of training benefit. PT Narrative Review 2017 Genetic Variation and Neuroplasticity: Role in Rehabilitation After Stroke. Journal of neurologic physical therapy : JNPT No new clinical finding. This is a narrative review of genetic variation and stroke rehabilitation. PT OT Narrative Review 2017 Consideration of Dose and Timing When Applying Interventions After Stroke and Spinal Cord Injury. Journal of neurologic physical therapy : JNPT For upper limb stroke rehabilitation, more repetitions did not clearly improve function, and measured gains did not translate into daily use. Walking after stroke and incomplete spinal cord injury may respond to intensive therapy, but timing and inflammation matter. PT OT Pilot 2017 A Neuromuscular Electrical Stimulation (NMES) and robot hybrid system for multi-joint coordinated upper limb rehabilitation after stroke. Journal of neuroengineering and rehabilitation After 20 sessions with a muscle-signal-controlled robot and electrical stimulation device, 11 people after chronic stroke improved upper-limb function scores and tracking accuracy. No control group means the combined intervention cannot be attributed to one component. PT OT Narrative Review 2017 Activity-Based Therapy: From Basic Science to Clinical Application for Recovery After Spinal Cord Injury. Journal of neurologic physical therapy : JNPT Intense, task-specific movement below the spinal cord injury is the core of activity-based therapy. The review also describes a movement-quality scale and three children with chronic SCI reported to change following locomotor training. PT Other 2017 Characterization of Volitional Electromyographic Signals in the Lower Extremity After Motor Complete Spinal Cord Injury. Neurorehabilitation and neural repair People with motor-complete cervical spinal cord injury often produced small intentional electrical signals in lower-leg muscles despite no visible movement. Signal quality varied, but some signals may be useful for future neuroprosthetic control. PT OT SLP Cohort 2017 Focused and divided attention abilities in the acute phase of recovery from moderate to severe traumatic brain injury. Brain injury People with moderate-to-severe TBI in acute recovery struggle to search visually when distracted, but can filter out irrelevant info if first given a spatial cue. They are also slower to confirm when a target is absent. Use location cues and allow extra time. PT Other 2017 University of the Witwatersrand physiotherapy undergraduate curriculum alignment to medical conditions of patients within Gauteng state health facilities. The South African journal of physiotherapy In Gauteng state hospitals, physiotherapists most commonly treated lower limb fractures, stroke and soft tissue injuries. The Wits curriculum covered almost all listed conditions; stress and laryngectomy were not explicitly covered. It allocated hours to common conditions. PT OT SLP Cohort 2017 A Short and Distinct Time Window for Recovery of Arm Motor Control Early After Stroke Revealed With a Global Measure of Trajectory Kinematics. Neurorehabilitation and neural repair Arm motor control recovery plateaued by 5 weeks post-stroke, while strength and clinical scores continued improving through one year. This suggests spontaneous biological recovery is largely complete early, whereas later gains likely reflect strength or compensation. PT Case Report 2017 Combined transcranial direct current stimulation and robotic upper limb therapy improves upper limb function in an adult with cerebral palsy. NeuroRehabilitation One adult with unilateral spastic cerebral palsy received 36 sessions of tDCS plus upper limb robotic therapy. His upper limb function and grip improved after therapy and remained improved at six months. This describes one person, not proof the combination works. PT Narrative Review 2017 Coupling Timing of Interventions With Dose to Optimize Plasticity and Participation in Pediatric Neurologic Populations. Pediatric physical therapy : the official publication of the Section on Pediatrics of the American Physical Therapy Association This is a narrative review about dosing and timing in pediatric neurologic populations. It has no original patient data or trial results, so there is no finding to act on. PT OT Cohort 2017 Therapy Use for Children With Developmental Conditions: Analysis of Colorado Medicaid Data. Pediatric physical therapy : the official publication of the Section on Pediatrics of the American Physical Therapy Association Colorado Medicaid infants and toddlers with more chronic conditions were more likely to receive physical or occupational therapy, at higher doses and spending. Those with developmental delays used more PT/OT than those with developmental diagnoses. PT RCT 2017 Effects of Pedalo® training on balance and fall risk in stroke patients. Journal of physical therapy science Stroke patients who did Pedalo balance training with conventional physical therapy improved some balance scores more than those doing treadmill training with conventional physical therapy. Fall risk scores went down in both groups, but not more with Pedalo. PT RCT 2017 Repetitive reaching training combined with transcranial Random Noise Stimulation in stroke survivors with chronic and severe arm paresis is feasible: a pilot, triple-blind, randomised case series. Journal of neuroengineering and rehabilitation Combining transcranial random noise stimulation, reaching training and outcome-triggered electrical stimulation was feasible in four severe chronic stroke survivors: no adverse events, all completed, and the study did not show added benefit over sham. PT RCT 2017 Comparison of the effects of core stabilization and chest mobilization exercises on lung function and chest wall expansion in stroke patients. Journal of physical therapy science In chronic stroke patients, core stabilization and chest mobilization exercises produced similar lung function and chest expansion changes. Neither was clearly better, though both groups improved some measures. PT Other 2017 Three-dimensional evaluation of postural stability in Parkinson's disease with mobile technology. NeuroRehabilitation An iPad app at the waist recorded greater sway in Parkinson's than controls across six balance tests, especially eyes closed on foam and tandem on foam. It can quantify direction-specific balance problems, not prove treatment works. PT OT Other 2017 Inter-rater reliability of kinesthetic measurements with the KINARM robotic exoskeleton. Journal of neuroengineering and rehabilitation When different robot operators set up the same participant, scores for matching arm movement were similar. The test can be used consistently by different operators, but it did not test treatment effects or daily function. PT Other 2017 Instrumented balance and walking assessments in persons with multiple sclerosis show strong test-retest reliability. Journal of neuroengineering and rehabilitation Wearable-sensor quiet-standing and extended Timed Up and Go measures gave similar results across days in people with MS, except some sway frequency measures. They did not match self-reported balance confidence or walking ability. PT OT Other 2017 The Impact of Shoulder Abduction Loading on Volitional Hand Opening and Grasping in Chronic Hemiparetic Stroke. Neurorehabilitation and neural repair Lifting the affected arm against gravity worsened hand opening in moderate chronic stroke and made severely impaired participants close their hand instead of opening it. Controls were unaffected. Total maximal grasping force did not change, but involuntary grasping force rose with loading. PT Other 2017 The effects of task-oriented training with altered sensory input on balance in patients with chronic stroke. Journal of physical therapy science Task-oriented training with altered sensory input, alternating firm and soft surfaces, added to usual physical therapy gave a higher Berg Balance Scale score than usual therapy alone. Limit-of-stability and balance-confidence scores did not differ. ← Previous PAGE 7 OF 78 Next →
