Nurses' care for older adults with leukemia receiving allogeneic stem cell transplant centers on frailty and geriatric assessment, central line infection prevention, chemotherapy monitoring, transfusion reaction checks, symptom control, and GVHD surveillance.

01Before transplant, nurses can perform or contribute to validated geriatric assessments, including the Geriatric Assessment in Hematology Scale, Geriatric 8, and Clinical Frailty Scale, to help judge an older patient's ability to tolerate conditioning chemotherapy.

02Central line care is a major nursing task during allogeneic HCT because about 9% of these patients develop CLABSI; prevention includes sterile dressing changes, chlorhexidine bathing education, flushing unused lumens, hand hygiene, and involving family caregivers.

03Nurses are expected to assess and grade GVHD, a complication reported in about 40-60% of HCT recipients and associated with 50% mortality.
