WHAT IT FOUND

Antenatal breastfeeding education and counseling raised mothers' confidence in breastfeeding.

The biggest gain came from model-based education plus counseling; counseling alone did not help.

Key findings

01Across 34 studies involving 4698 pregnant participants, antenatal breastfeeding interventions had a positive pooled effect on postpartum breastfeeding self-efficacy compared with routine care.

02Model-based breastfeeding education plus counseling had the largest pooled effect (2.644, 95% CI 1.650 to 3.639), while model-based counseling alone did not show a significant effect (0.114, 95% CI -1.020 to 1.249).

03The studies varied enormously (I² = 95.016%), and intervention type explained only 16% of the relationship.

What it does not show

The studies varied enormously (I² = 95.016%), so the pooled result may not apply to any one clinic or patient group. Only English and Turkish articles were included, so results may not generalize elsewhere. The review evaluated interventions started during pregnancy, so it does not address what works after birth. Simulation education was tested in only one included study, so its apparent lack of impact is not reliable. Gray literature and non-peer-reviewed reports were not quality assessed.

The easy way to misread this

Do not read this as evidence that antenatal breastfeeding education improves actual breastfeeding duration or infant health. The pooled positive result was for breastfeeding self-efficacy scores, and the studies varied enormously.
