WHAT IT FOUND

Frail or pre-frail older adults using a WeChat platform plus routine community care had better frailty status, mood, sleep, grip strength and quality of life at 12 weeks.

The platform bundled exercise, nutrition, education and monitoring, so single parts cannot be separated.

Key findings

01At 12 weeks, the intervention group had 27 pre-frail and 0 frail older adults, while the control group had 20 pre-frail and 5 frail (P = 0.020).

02At 12 weeks, mean quality of life was 557.15 in the intervention group and 463.54 in the control group on a 0 to 800 scale (P < 0.001).

03The platform had a mean usability score of 87.96 on a 0 to 100 scale, and no adverse events were reported during the intervention.

What it does not show

The study was quasi-experimental, with groups allocated by community rather than randomly, so baseline differences between communities may explain some results. Only 52 of the 60 recruited older adults were analysed, and 8 dropped out, so the sample was small. The intervention was a bundle of routine community care, health assessment, health education, multicomponent exercise guidance, nutritional guidance, reminders, check-ins, and nurse follow-up, so the contribution of the mobile platform or any single component cannot be separated. Participants had to be proficient with mobile phones and WeChat, able to read Chinese independently, and free of cognitive, visual, hearing, or severe medical problems, so results may not apply to older adults with communication, sensory, or cognitive impairments. Only participants were unaware of allocation; the paper does not report blinding of outcome assessors. The study followed participants for 12 weeks and did not test long-term effects or sustained platform use; usage declined toward the end. The paper reports several outcomes but does not clearly identify a primary endpoint, so some significant findings may be secondary.

The easy way to misread this

Do not conclude the WeChat platform alone improved frailty. The intervention group also received routine community care, health assessment, health education, multicomponent exercise guidance, nutritional guidance, first on-site nurse exercise guidance, reminders, check-ins, and follow-up calls or home visits, so the effect of any single component cannot be separated. Allocation was by community rather than random, and the study analysed 52 older adults over 12 weeks.
