Stroke Caregiver Depression: Qualitative Comparison of Treatment Responders and Nonresponders at 1 Year.

WHAT IT FOUND

The stroke caregiver problem-solving program made no difference to one-year depression compared with control.

Among caregivers who received it, those whose depression improved used active coping; those who did not improve focused on loss and used few strategies.

Key findings

01Problem-solving methods were used by 10 of 12 responders and 6 of 13 non-responders.

02Responders tended to talk about how life improved and restructure life, while non-responders focused on what was lost.

03Nine of 13 non-responders mentioned counselor support, but only three also mentioned a coping strategy.

What it does not show

The qualitative sample was small and purposive, with 25 caregivers selected from one treatment arm, so it cannot show what works for most stroke caregivers. The wait-list control group was not included in the qualitative interviews, so the study compares only caregivers who received the intervention. The parent trial found no difference in one-year depressive symptoms between treatment and control, so responder status within the treatment arm cannot be treated as evidence that the program works. More men and spouses were in the non-responder group, which may mix caregiver role differences with response status. Coping strategy use was self-reported 9 to 10 months after counselor contact ended, and no contact after the six-month assessment, so endurance of strategy use could not be assessed. Findings are themes from interviews, not measured effects of the intervention.

The easy way to misread this

Do not conclude that the problem-solving program reduces caregiver depression. The parent trial found no difference in one-year depression between treatment and control. This study only describes what responders and non-responders said about coping.
