Reduction of Scheuermann's deformity and scoliosis using ScoliBrace and a scoliosis specific rehabilitation program: a case report.

Christopher M Gubbels, Paul A Oakely, Jeb McAviney and 2 others

PMID 30858656

WHAT IT FOUND

Kyphosis fell from 79° to 63°, scoliosis from 30° to 25°, disability and pain were lower after a custom brace, heel lift, ScoliRoll stretching and strengthening exercises in a 26-year-old man.

Single case, not proof the brace caused improvement.

Key findings

01At 4.5 months, the thoracic kyphosis angle was 63°, down from 79°, and the lumbar scoliosis angle was 25°, down from 30°.

02The Oswestry Disability Index score was 18/100 initially, 4/100 at 4.5 months, and 6/100 at 12.5 months.

03The patient reported average pain 4/10 and worst pain 7/10 initially; at 4.5 months he was pain free on average with occasional 2/10 pain.

What it does not show

This is a single case report, so it describes what happened to one person and cannot show that the treatment worked. Several treatments were given at once: the ScoliBrace, a 6 mm heel lift, ScoliRoll stretching and strengthening exercises. The paper says the contribution of each component cannot be established. The patient was not as compliant with the rehabilitation protocol as with bracing, using the ScoliRoll once per week and occasionally doing exercises. There was no comparison group. No in-brace lateral radiograph was taken, so the brace's immediate effect on the kyphosis was not measured. The patient was a young adult male with likely post-Scheuermann's presentation, so results may not apply to older adults, degenerative kyphosis, or people with limited mobility, lower bone density or sarcopenia. Follow-up was 4.5 and 12.5 months, and the authors say longer follow-up is needed to see whether effects are maintained and whether bracing affects trunk muscles.

Declared interests

There was no funding. JM is the owner and director of ScoliCare, a company that provides treatment and educational products for practitioners and patients with scoliosis, and JM created the ScoliBrace orthosis and the ScoliRoll tool. BTB is a paid employee of ScoliCare.

The easy way to misread this

Do not read the angle reductions as proof that the brace caused improvement. The patient received a brace, a heel lift, ScoliRoll stretching and strengthening exercises together, and this was a single case report without a comparison group.
